Are you prepared to register for M.E.D.S. After-School Experience (M.E.D.S. ASE)? Sign up in-person or online is made easy.

We recommend online registration. In-person or mailing in is an option.  Registration forms can be printed and returned to P.O. Box 300131 | Houston, Texas 77230. ($80.00 one-time registration fee must be included at the time of submission using money order, cash or cashier’s check made out to M.E.D.S. After-School Experience)

Registration Checklist

M.E.D.S. ASE believes the most important component in this after school experience is our students.

Signing up in-person or online is made easy. Have the following items ready to complete registration:

  1. Student’s school ID number
  2. Immunization records (if not an H.I.S.D. student)
  3. $80 registration fee (must be paid at time of registration – non-refundable)
  4. Driver’s License number, phone number and address for all persons authorized to pick up – An emergency contact that is not a parent or guardian (also need Driver’s License number, phone number and address)
  5. Medical insurance information – Address and phone number for pediatrician and hospital of choice in case of emergency – Are there any special accommodations or medications needed during M.E.D.S. ASE time? A M.E.D.S. ASE representative will follow up for more detail. – Will you be applying for financial assistance? If so, you will need your tax information.

Days/Times Offered:

The after-school experience is open Monday-Fridays, 4:00P-6:30P school days only. Holiday times and location varies.

After-School Experience Location and Cost

Facility:

The after-school experience is hosted at middle school campuses.

$80.00 one-time registration fee

$100.00 per week in full.

$350.00 per month in full.

Late Pick-up is 6:30P-7:30P and is $30 additional fee and paid during registration. Unplanned late pick-up is $15 for up to 30 minutes and $30 after 30 minutes (paid upon pick-up). If running late, please call a M.E.D.S. ASE staff representative.
